Instructions to Make an Online Appointment Using Appointment Quest
Self-scheduling can be difficult the first time you try it, so here is the process in print. If you just want to give it a go, then Make an Appointment.
- From the Village Family Clinic Appointment Quest page, you can either select the appointment you want or select the Login tab from the bar just under the logo.
- Either of these selections will require new patients to enroll with some required information before they login, or will prompt the returning patients to use their login and password.
- After enrolling, you will be able to select from three appointment options:
- Physical Exam Appointments – these are for complete physical exams, gynecological exams and well child exams.
- Complex Medical Visit – these are longer visits for more complex or multiple medical issues. If you have 3 or more issues to discuss or feel you will need extra time, please use this appointment slot.
- Same Day Appointments – these are short appointments for urgent or acute medical visits. These are 15 minute, same-day visits
- Select the month and day from the calendar and a group of times will appear at the bottom of the page. Select the time you want to come in.
- The next page will ask for an activation code from the email sent after you enrolled. This is for security purposes.
- Review and confirm screen allows you to verify patient information and add the reason for the visit, if desired. Select “Make Appointment” to confirm or “Cancel” to start over.
- We will send you confirmation by email and a reminder email 24 hours before physical exams.
